III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the inspection <br />and the facts or evidence supporting the enforcement action. <br />This was the complete quarterly inspection for the 1" Quarter, 2002. The inspection was conducted by Dan <br />Mathews of DMG, accompanied by Scott Wanstedt of BME. Weather was clear and mild, surface soils in <br />most locations were moist from light precipitation several days previously. Most areas were snow free, <br />with only a scattered few remnant snow patches on shady north slopes. Field inspection was conducted <br />Mazch 26 and March 27, and records check was conducted on Apri14. <br />One Notice of Violation was issued as a result of the inspection. NOV CV-2002-004 was issued 3/27!02 at <br />Spm, for "Failure to maintain sediment pond primary spillway..." at Sediment Pond RP-2/3. A description <br />of the pertinent facts is included under "Hydrologic Balance" section of this report. <br />Availability of Records <br />Required records were on file and up to date through the 4's Quarter, 2001. See attached "DMG <br />Availability of Records" form. <br />Roads <br />Refuse haul road and various access and light use roads in the long-term facility areas were inspected and <br />generally found to be in good condition. Maintenance was needed in various locations due for the most part <br />to sediment accumulation or minor erosion associated with spring thaw and recent precipitation. Locations <br />where maintenance is warranted include the following: <br />• Refuse haul road ditch outlet on southeast side of road, west of refuse areas and adjacent to the fast <br />haulroad topsoil pile. Maintenance is needed at several of the rock checks to clean out sediment and <br />prevent drainage from cutting azound the checks. Additional rock should be placed along sections of <br />the down-drain where minor cutting has initiated. <br />At the next rocked outlet on the same side of the road, the upper rock check was largely washed out, <br />and needs to be re-built, or alternatively, rock could be placed along the upper I S foot section of the <br />down-drain to line and protect the ditch. <br />• At the second outlet southwest of Transfer #1, on the west side of the haul road, the diversionberm at <br />the outlet location needs to be built up to ensure that ditch flow is diverted at the outlet. <br />• Minor rock placement is warranted at the ditch outlet at the south end of the large road fill north of the <br />CR-65 junction, on the east side of the road. <br />• Grading is warranted along the haul road ditch in the vicinity of the entrance to "Treasure Island" <br />storage yard, to better define the ditclvberm. The segment needing maintenance extends from <br />immediately north of the storage yard entrance road to the location of the metal gates farther south on <br />the haul road. <br />• _ On the conveyor access road, several culverts require minorcleanout at the outlet end, including the <br />first culvert north of CR-65, culvert at south end of Transfer #I fill, culvert beneath first fill north of <br />Transfer #1, the culvert further north adjacent to Hydrant #20, and the culvert directly east of the north <br />end of the slot storage bench. <br />• Also on the conveyor access road, the longriprapped ditch outlet downdrain located approximately <br />300 yazds south of Conveyor Transfer #2 exhibited minor cutting azound theriprap at the lower end, <br />and repairs are warranted. <br />
