ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_19An-yea <br />INSPECTION DATE: I(U9/+u INSPECTORaS INITIALS: ~~ <br />OBSERVATIONS <br />The Division conducted a ground water sampling inspection with Mr. Lawrence <br />Myers of CC&V on 10/9/06. The main purpose of the inspection was to collect <br />a sample from the ground water monitoring location at CRMW 3A&3B.After the <br />wells were purged in accordance with EPA Guidelines; a split sample was <br />collected for WAD cyanide analysis and metals. Metals for metals were <br />neither preserved or filtered, they were properly iced and delivered to the <br />health department at 10:00 a.m on 10/10/06. EPA does not recommend <br />acidification of samples if they are delivered with in 48 hours after <br />samples were collected. <br />After the samples were collected we looked at AG 1.5, it was dry. Both the <br />South and 4 inch pipes of the under drains were discharging at the time of <br />the inspection. All the lower ponds and storm water detention basins were <br />found to be in good repair <br />Inspected the leach pad areas and collected the following readings from the <br />high and low volume solution collection transducers for the pregnant <br />solution collection system. <br />PHASE I High volume solution collection transducer # 2 was recorded at 22.7 <br /># 3 was at 23.2. Low volume collection transducer # 1 was recorded at .15 <br />and # 2 was at .51. <br />PHASE II AND III High Volume Solution Collection transducer # 4 was <br />recorded at 14.8 # 5 was at 15.3 and # 6 was at 15.3 Low Volume <br />collection transducer # 1 was at .15 and # 2 was at.51. <br />Phase IV High Volume solution collection transducer # 7 & 8 were under <br />going routine maintain ace, # 9 was at 12.0 and 3 10 was at 12.3.
