(Page 2) <br />MINE ID # OR PROSPECTING ID # : ~~-~ orin-~aa <br />INSPECTION DATE: 12-n.5_n~ INSPECTORS INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress, inspection of the site on 12-05.02. Present <br />during the inspection were Phil Barnes and Mark Tidquist of CC& V. The Inspection began at the sub-station area <br />where, the Phase IV construction work was viewed, from that location. Backfill and grading along the Phase IV <br />(B) AND (C) areas were continuing. Leak Collection gravel was being placed on top of the primary liner of the <br />PSSA along the West side, at the time of the inspection. All geo membrane liner deployment has ceased until <br />springtime. The appropriate storm water protection, protecting the lined areas from the unlined areas was <br />properly placed, no major problem noted. <br />Inspected the phase l AND II High and Low Volume Solution Collection areas and recorded the following <br />readings from the Transducers. <br />Phase i High Volume SOLUTION COLLECTION TRANSDUCER #1 WAS RECORED AT 38.5, #2 WAS AT 41.7 AND 3 WAS <br />AT 41.2. <br />Phase I Low Volume SOLUTION COLLECTION TRANSDUCER # 1 WAS RECORDED AT .74 AND # 2 WAS AT .ib <br />Phase II High Volume SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 16.4, # 5 WAS AT 15.1 AND # 6 <br />WAS AT 16.8 <br />Phase II Low Volume SOLUTION COLLECTION TRANSDUCER # i WAS RECORED AT .68 AND # 2 WAS AT .12. (ALL <br />READINGS ARE IN FEET OF HYDROSTATIC HEAD) <br />Inspected the external pond, it was holding some water, which appeared frozen. The following readings were <br />collected from the LOW VOLUME SOLUTION COLLECTION TRANSDUCER. # 1 WAS RECORED AT 15.3 INCHES AND # <br />2 WAS AT 17.2 INCHES OF HYDROSTATIC HEAD. Water level was at approximately 8.38 feel. <br />Inspected the under drain-area. The disturbance associated with the construction of the pump back system is <br />now graded and should be finalized with in the next week or so, according to Mark Tdquist. No problem noted. <br />Both the 4-inch and south under drainpipes were discharging a trickle.
