(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~1-~ vBn-gee <br />INSPECTION DATE: 5-~b-0'~ INSPECTOR'S INITIALS: ~- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the slte on 5-26-03. The main <br />purpose of the inspection was to over see the placement of the caisson foundation for the Phase IV pregnant <br />solution collection area. Present during the Inspection were, Terry MANDZIAC AND Mark Tidquist of CCBV. The <br />are had received some rain the night before, which had collected in the Low Volume solution collection area of <br />the Phase tV. the water was being pumped at the time of the inspection in anticipation of the placement of the <br />caisson foundation. Terry explained the progress and stated that once the water was pumped out, offer the two <br />feet drain cover is placed the caisson foundation will commence. The water collected from the rain the night <br />before was being pumped out of the pond at the time of the Inspection. While waiting for the water to be <br />pumped, inspected the Phase I AND II pregnant solution collection systems and recorded the following <br />readings from the Phase I AND I HIGH AND LOW VOLUME SOLUTION COLLECTION SYSTEM TRANSDUCERS. <br />PHASE I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS RECORDED AT 33.1 # 2 WAS AT 36.2 AND # 3 <br />WAS AT 35.7. LOW VOLUME TRANSDUCER # I WAS RECORDED AT .37 AND # 2 WAS AT .26. PIEZOMETER WAS AT 40.0 <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 23.5 # 5 was at 16.8 and # 6 <br />was at 23.5, PIEZOMETER WAS RECORDED AT 28.1. LOW VOLUME TRANSDUCER # 1 AND 2 DIGITAL READOUTS WERE <br />NOT FUNCTIONING PROPERLY. THE TRANSDUCER PUMP # 2 WAS ON, SINCE THE DIGITAL READ FOR # 2 WAS <br />REMOVED FOR REPAIR, AND THE # 1 READ OUT WAS SHOWING 11.48 # 1 PUMP WAS TURNED ON MANUALLY. Terry <br />Mnadziac contacted the process crew to find out what was going on, and they responded that due to an <br />electrical malfunction, they had to remove the unit for repair. In the future however, when such things happen, <br />CC&V MUST TURN BOTH PUMPS ON, TO MAKE SURE THAT THE HEAD IS NEVER EXCEEDS TWO FEET OF HEAD. If the <br />pregnant solution head had been around 40 feet of head, the Division would have had no choice but to note it <br />as a possible problem. However, given the many years of collecting readings from the area, we have <br />established that as long as one of the pumps is operating at a!1 times and the head at the pregnant solution <br />area Is below 40, it is highly unlikely for the head to exceed 2 feet at the low volume solution collection area. In <br />addition, offer the # 1 pump read out even offer the pump was turned on was still showing 11.47, even offer it <br />had pumped all of the water, it was also assumed that it too was malfunctioning. Affer the solution was pumped <br />out, the # 1 pump was turned off. On 5-27-03, I contacted Lawrence Meyers, who usually collects the readings <br />for CC&V and informed him of my findings. He informed me that all were functioning as designed on Friday <br />when he collected the readings and it probably happened over the weekend and that they are going to have <br />(CCBV) WAS GOING TO HAVE A MEETING today, 5-27-03, TO SEE HOW TO BETTER COMMUNICATE SUCH events, so <br />the same confusion will not happen again. <br />I & E Contact Address <br />NAME: MR.
