ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-I9Rn_~ae <br />INSPECTION DATE: h/~/Od INSPECTOROS INITIALS: -BK- <br />OBSERVATIONS <br />The Division conducted a construction progress and monitoring inspection of <br />the site on6/3/04 Present during the inspection were Mark Tidquist and Tim <br />Comer of CC&V. <br />Inspected the Phase IVC leach pad extension area. Clay and Geomembrane <br />deployment was being conducted along the South West corner of the expansion <br />area. The deployed liner was walked and inspected; the deployed liners <br />appeared to meet the approved specifications. QA/QC personnel from Golder <br />Engineering, the design and QA/QC firm, were on site at the time of the <br />inspection, over seeing the deployments. <br />Inspected the Phase I, II, III and IV leach pad areas and recorded the <br />following readings from the Pregnant solution collection systems for the <br />high and low volumes. <br />Phase I high volume solution collection transducer # 1 was recorded at 22.3 <br /># 2 was at 25.6 and #3 was at 24.S.LOw volume Transducer # 1 was at .16 and <br /># 2 was at .61. Piezometer for the pond was at 34.0 feet <br />Phase II & III high volume solution collection transducer # 4 was recorded <br />at 25.4 #5 was at 20.1 and # 6 was at 25.8. Low volume transducer # 1 was <br />at .06 and # 2 was at .45. Piezometer for the pond was recorded at 28.8 <br />feet. <br />Phase IV high volume solution collection transducer # 7 was recorded at <br />19.4 # 8 was at18.5 #9 was at 19.7 and # 10 was at 20.1. Low volume <br />transducer # 1 was recorded at 13.3 and # 2 was at 11.8 inches of <br />hydrostatic head. (ALL OTHER TRANSDUCER READINGS ARE IN FEET OF HYDROSTATIC <br />HEAD). <br />Inspected tha eternal pond, it was holding some water. While in the area, <br />recorded the following readings from the Low volume solution collection <br />transducers. Low volume solution collection transducer # 1 was recorded at <br />11.8 and # 2 was at 9.2 inches of hydrostatic head. <br />Inspected the under drain area, all lower ponds with the exception of the <br />lowest pond were dry.
