<br />• (Page 2) • <br />MINE ID # OR PROSPEC'T'ING ID # M-81-080 <br />INSPECTION DATE 3/4/98 INSPECTOR'S INITIALS AJW <br />OBSERVATIONS <br />This was a normal monitoring inspection conducted by Tony Waldron of DMG along with Mike <br />Tezak of Tezak Stone. The site is located just south of U.S. Hwy 50 approximately 4 miles <br />east of Cotopaxi, Colorado. The site is a 110 operation located in an old upland alluvial <br />terrace. <br />The permit boundary was clearly marked with small boulders and the identification sign was <br />properly posted at the mine entrance and contained the required information. A small <br />disturbed area just east of the affected area had been reclaimed and was looking good. This <br />area was the subject of an earlier enforcement action. Mining activities were occurring in <br />the pit located near the west central part of the permit area. Topsoil stockpiles were <br />located along the southwest and northwest corners of the permit area and were properly stored <br />and stabilized.
