(Page 2)
MINE ID # OR PROSPECTING ID # M-94-080
INSPECTION DATE 02/02/00

INSPECTOR'S INITIALS ESC
This was an inspection of the Berthoud Sunset Quarry conducted by Erica Crosby of the
Colorado Division of Minerals and Geology. Neil Sprague was present during the inspection.
Weather conditions on site were clear and warm. Ground conditions were dry.
The Berthoud Sunset Quarry is located approximately 2 '~ miles south of Carter Lake in
Larimer County, Colorado. The 110 permit was issued to Arkins Park Stone Corp. on September
1, 1994. The permitted affected area is 9.9 acres in size. No mining was taking place
during the inspection.
The current affected area is roughly 2-acres in size. Disturbance consists of the existing
quarry, storage and work deck, riprap piles and waste rock piles. The stone quarried from
this site was used in the construction of the South Park Meadows Mall. Quarrying is
following the natural dip of the rock, so a highwall does not exist. Reclamation will mainly
entail regrading the workbench, the riprap and waste rock piles. Therefore, the current
financial warranty in the amount of $4,635.70 is sufficient to complete reclamation of the
site.
A mine identification sign was not posted at the entrance to the site. In addition, permit
boundary markers could not be located in the field. The operator will need to post a mine
identification sign and permit boundary markers as required by Rule 3.1.12.
I & E Contact Address cc: Carl Mount, DMG
^ CE
NAME Neil Sprague ^ BL
OPERATOR Arkins Park Stone Corp. ^ FS
STREET 5975 NCR 27 ^ HW
CITY/STATE/ZIP Loveland, Colorado 80538 ^ HMWMD (CH)
^ SE
^ WQCD (CH)
^ OTHER
cERT1FIEp uux.t~to~ ~5 3~0
RETURN RECEIPT
