., <br />L <br /> <br />(Page 2) <br />MINE ID f OR PROSPECTING ID f M-80-244 <br />INSPECTION DATE 8 12 94 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />'t ~I~. <br />Aw-i~apact~en Cresson Project Phase 1 pad area was inspected. An inspection of the Victor <br />Pad was also conducted, including cursory inspection of the Ironclad and Globe Hill pits and <br />the Squaw Gulch area. <br />The Phase 1 underdrain sump has been excavated to the approved grade and has been lined with <br />VLDPE. At the time of this inspection water was being pumped from the sump past the Carlton <br />Mill tails and discharged into lower Arequa Gulch. There was water flowing into the sump <br />from both the 24" solid pipe and from the 24" perforated pipe. The water entering the sump <br />from the perforated pump was cloudy due to a heavy load of suspended eolide. The operator <br />was instructed to put some sediment controls in place at the pumped water discharge point to <br />filter the suspended eolide. The controls utilized should be in accordance with the Beet <br />Management Practices described in the storm water permit for this site. The operator stated <br />that sediment controls would be installed commencing immediately. During Phase 1 leach <br />operations, the operator intends to maintain a pump in the Phase 1 underdrain sump with an <br />approximately 300 gpm capacity. This should be orders of magnitude more than adequate to <br />handle underdrain flows during normal operations, and it would not be difficult to increase <br />pumping capability in the sump if it should become necessary. <br />The Phase 1 toe berm foundation was inspected. The underdrain installation was complete in <br />this area. The contractor was placing and compacting select structural fill preliminary to <br />installation of geotextile and geomembrane. Once the geomembrane ie installed over the <br />underdrain in this area, and covered with protective fill, the contractor can begin <br />construction of the main portion of the toe berm. <br />The current upstream extent of the underdrain was inspected. The 4" and perforated 24" pipes <br />have been capped at the upstream ends and temporarily buried. A VLDPE lined temporary sump <br />has been constructed to collect Arequa Gulch flows, primarily from the Arequa Gulch spring. <br />A screened riser has been attached to the solid 24" pipe in the temporary sump, and the <br />Arequa Gulch flows are conveyed to the Phase 1 underdrain sump through the solid pipe. <br />The contractor is placing fill for the processing plant access road just below the current <br />upper extent of the underdrain. An area of fill containing tree roots and other deleterious <br />material was noted. The contractor was instructed to remove this unacceptable fill. The <br />removal was completed before we left the site. <br />in place lifts of primary underdrain were inspected. The underdrain fill exposed for <br />inspection was very durable, and contained few if any fines. The contractor was breaking up <br />oversize using a small excavator equipped with a jack-hammer attachment. <br /> <br />The main topsoil stockpile was inspected. <br />controls (hay-bales) had been washed out <br />instructed to repair these ditch controls, <br />water controls through-out the site. <br />It was noted that some roadside ditch sediment <br />and were non-functional. The operator was <br />and to perform continuous maintenance on storm <br />The Victor Pad was inspected. An area to the north and below the pad that was reclaimed two <br />seasons ago ie doing very well with a good stand of grasses established and no significant <br />erosion noted. An area on Victor Pad phase 5 was examined were material had been placed very <br />near the edge of the pad. The material could block and pond etormwater flows at the <br />perimeter of the pad, and potentially result in excursions of process fluid off the lined <br />area. The .operator was instructed to remove all material from near the edge of the pad. <br />This corrective action was initiated while we were still at the site, and was completed by <br />the time the Division inspected the Bite on 8/14/94.
