(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1978-091 UG <br />INSPECTION DATE 6/29/06 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of Hard Rock and Metals 110d permits. The operators <br />named on page one were present during the inspection. Also onsite during the inspection were several people representing the <br />Gault Group (operator's consultant), and BLM and EPA, specifically: Steve Glass and Ivan Messinger (Gaut) Group); Dave <br />Lazorchak, Barbara Hite, Lynn Lewis, and Bill Carey (BLM); Jerry Cross and Valois Shea (EPA). <br />Inspection of the permitted area only consisted of the 6 level. The site was not active at the time of the inspection. There was <br />only a small stockpile of ore (against the cutslope N of the portal), and no sacked ore stored onsite. There were a few pieces of <br />underground equipment parked at the edge of the pad. The crusher and conveyor were still set up in the same location, as <br />were the generator and compressor. <br />About 50 cubic yards of new waste rock was placed at the SE end of the pad, not against the hillside and not in the gulch. A <br />trash pit was excavated into the waste rock, but it did not appear to contain hydrocarbon containers or lead/acid batteries. The <br />fuel and oil is now adequately stored with secondary containment and protection from precipitation. This storage should be <br />carefully used, maintained and regularly monitored for spills. <br />The outer edge of the pad was bermed and all drainage from the pad surface reported to the small depression just S of the <br />portal, as intended. There was no discharge from the portal. <br />The cutslope does not appear to have eroded or sloughed since the last inspection. The N permit boundary was still marked <br />along the top of the cutslope. Several boundary markers are within a few feet of the top of the cutslope, and they will be <br />monitored to ensure that there is no offsite disturbance. The borrow area at the NE end of the site has not been stabilized <br />either, but there has been no visible failure of that slope to-date. <br />The outslope of the dump has been stable, with no failure of the benches and no material moving down the gulch. The <br />operator will not be further disturbing the dump face, since all new dump rock material will be placed in lifts on the pad surface. <br />Given the apparent difficulty in getting revegetation to grow on the mineralized, clayey dump slope, the operator was <br />encouraged to begin revegetation efforts there. <br />An upcoming modification to the permit was discussed, involving adding some land to the permit adjoining the N side of the 6 <br />level. That filing should also address changes to the approved onsite activities, which still are not approved for the crusher. <br />No inspection was performed of the 3 level portal and dump. <br />An informal meeting and inspection of the proposed drilling location at the bottom of the valleywas performed, with most of the <br />persons named above stiff in attendance. The operator obtained a BLM well permit, but no permit was required from DMG. <br />No further items were observed during the inspection.
