• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-77-410 <br />INSPECTION DATE 06/16/99 INSPECTOF.'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during a succession-of-operator inspection of this 2- <br />acre 110 site. <br />1) Signs and boundary markers are in place as required by the rules. Most of the boundaries <br />are defined by the natural boundaries of the site, but fencing is in place on the south <br />side. <br />2) The two acres of the site include five buildings, two ponds (one a stormwater settling <br />pond, the other a treatment pond for runoff from the mine), one mine adit, and storage <br />space for equipment and vehicles. Some overburden and topsoil has been stored along the <br />southern boundary. <br />3) The treatment pond has approximately 1.5' of freeboard at this time, and appears to be <br />clear and free of any contamination. The stormwater settlement pond has 1' of freeboard, <br />and also appears to be clean and in good shape. No tears were noted in the pond liners, <br />and no leaks were noticed from any of the pipelines leading between the ponds, the mine, <br />and the final NPDES discharge point downstream. None of the water appears to be going over <br />tailings or stockpiles. <br />4) One material/overburden stockpile was in place on the site, next to a smz~ll 25'x 25' hole <br />excavated next to the covered mine track out to the nearby storage building. It appears <br />to be large enough to fill the hole next to it. <br />5) Two of the buildings on the site are residences, and one is a mobile home used as an <br />office. The other two buildings are a storage warehouse, which contains the core drilling <br />samples, an air compressor, and some other unused equipment, and the equit~ment shop, which <br />also has a controlled access to the mining adit. The equipment shop contained barrels of <br />trash, disused respirators and mining hats, coats and boots, and several disused <br />generators, as well as piping. No trace of chemicals which would qualify the site for DMO <br />status were found. <br />6) The external entrance to the adit was covered by aluminum doors which had been nailed shut <br />with a wooden bar along the seam, and the internal entrance requires the keys to both the <br />garage door and the internal entry. A company employee lives on sight anti keeps an eye on <br />the entrance to prevent illegal entry. <br />7) The wastewater treatment plant which was planned for in 1993 has still not, been installed. <br />If the site ever begins processing ore material, this will need to be remedied, in <br />accordance with your CDPHE permit. <br />8) Plans for the final reclamation of the buildings on the site have not been included in the <br />reclamation plan. A Technical Revision will need to be submitted to leave the buildings <br />in place after the site has been reclaimed; otherwise the.cost of demolition must be <br />calculated and included in the required bond. Examples of~acceptable letter(s) from the <br />County are enclosed. (PB1) <br />9) The two on-site core drilling pads have been reclaimed. MP9 is now a graved ed-over parking <br />lot area, and MP10 is a revegetated site south of the road. <br />No other problems noted at this time. <br />
