__ <br />INSPECTION REPORT - PAGE 2 <br />FILE N0. 76-32 <br />DATE: April 12,'1984 <br />OBSERVATIONS OF IMPORTANCE: <br /> <br />1. It appears that the total acreage disturbed including shop facilities <br />exceeds 14 acres. <br />2. They have not complied with inspection report requests of August 19, <br />1983. (See attached) <br />3. Present financial warranty is X3,500.00. <br />RECOMMENDATIONS: <br />1. Recommend to the MLR Board that they find a violation and issue a cease <br />and desist order until the company complies with inspection requests of <br />August 19, 1983 and have the company supply maps showing existing disturbance. <br />2. In addition, have the company demonstrate that they have sufficient <br />topsoil fines to complete the approved reclamation plan. <br />3. Financial warranty should be adjusted to reflect 6" of fines replaced on <br />site over the disturbed area. Therefore, an additional cost of <br />.5' x 43 560 cu d x 14 ac = 11,293 cu yd x X1.50 cu yd = X16,960.00 <br />cu y ac <br />for earth work should be added to the existing `f3,500.00 for a total of <br />X20,460.00. <br />If the acreage, from new maps, shows additional disturbance above 14 acres, <br />then the financial warranty should be re-adjusted to reflect importation of <br />topsoil/fines. There does not appear to be sufficient fines to cover the <br />present disturbance to a depth of 6" at this time. <br />4. A permit amendment should be submitted by the operator to reflect <br />existing conditions and future plans as soon as possible. <br />/th <br />Attachment <br />Doc. No. 1309 <br />
