<br />(Page 2) • <br />MINE ID # OR PROSPECTING ID # M-91-134 <br />INSPECTION DATE 5-14-99 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS BK <br />The Division conducted a monitoring inspection of the site while in the area. The site used <br />to be an old leach pad which the current operator CC&VG has converted the permit into a 112 <br />Construction Material Permit with a token bond of $7,530.00. Three sand and gravel operators <br />have leased the area from CC&VG to crush and process the spent ore for construction material <br />byproduct. CCEVG has also submitted A TR to place two concrete batch plants within the permit <br />area. Present during the inspection were, Mr. Phil Barnes and Mr. Scott Lewis of CC&VG. There <br />was a small amount of snow on the ground. Permit sign and boundary markers were properly <br />displayed. There were large quantities of processed material scattered through out the permit <br />area, There were three crushers and a few screens located along the northern part of the <br />permit area. A couple of dozers and front end loaders were noted scattered through out the <br />permit area. <br />The approved reclamation plan for the site is for CC&VG or their representatives OR OPERATORS <br />representatives to remove as much material as possible and stabilize the remainder of the <br />spent ore that they are unable to process for what ever reason. Inspected the lined ponds <br />located at the toe of the material being processed.
