• (Page 21 <br />MINE ID ~ OR PROSPECTING ID ~ <br />INSPECTION DATE 3126/97 <br />M-76-7 7 6 <br />OBSERVATIONS <br />• <br />INSPECTOR'S INITIALS WHE <br />This complete inspection was conducted by Wally Erickson from the DMG Durango office, 484 <br />Turner Drive, Building F, Suite 101, Durango, CO 81301, phone (970) 247-5193. Glen Williams <br />and Robert Schwind, Cotter Corporation Nucla office, accompanied the DMG inspector throughout <br />the inspection. The SM-18 Mine is located approximately 2 miles north of Uravan, below the north <br />rim of Spring Creek Mesa on the San Miguel Bench, approximately 5900' elevation, in portions of <br />the SE 1 /4 of Section 21, portions of the SW 1 /4 of Section 22, portions of Section 27, and <br />portions of the NE 1/4 of Section 28, T48N, R17W, NMPM, Montrose County. Operation is <br />approved for 14.9 acres affected area. DMG holds 514,000 financial warranty. Operations <br />appeared dormant during the time of this inspection. <br />Records -Permit was issued on 10/31 /79. In 1981, operations went into Temporary Cessation (TC) <br />due to declining market conditions. On 8/22/85, DMG approved 5-year extension of TC status. <br />On 8/31/90, DMG approved Technical Revision 1 (TR-01-, revising mine status from TC to <br />intermittently active. DMG records do not indicate additional modifications to the permit. <br />Signs & Markers -Mine identification sign and affected area markers were observed. Mine entrance <br />road was well maintained. <br />Facilities Area -Public access to mine portal was controlled by locked metallic doors. Shop building <br />and fresh water tank were well maintained. Waste rock stockpile was bordered on east and west <br />sides by topsoil stockpiles. Vegetative cover for topsoil stockpiles was established, no evidence <br />of topsoil erosion by runoff or wind. Drainage from waste rock stockpile passes through silt fence <br />prior to leaving site, silt fence was intact and well maintained. Mine is periodically dewatered, mine <br />water is pumped to evaporation pond near topsoil stockpile. Evaporation pond was lined with <br />impermeable fabric and surrounded by 12' high woven wire fence. Pond is anon-discharging <br />structure, no NPDES permit exists. Water level was approximately 24" below inlet elevation of <br />emergency spillway. Operator stated that mine produces sufficient water to maintain water level <br />within pond. Evidence of erosion/sedimentation to the affected areas and to the surrounding areas <br />was not observed. <br />I & E Contact Address <br />NAME David H.
