(Page 2) <br />MINE ID # OR PROSPECTING ID # NA <br />INSPECTION DATE 9/5/03 <br />INSPECTOR'S INITIALS WHE <br />This inspection occurred to verify preparations of the tunnel and bulkhead forms for a bulkhead in the Koehler Mine, <br />prior to concrete pour. DMG does not have a reclamation permit for the Koehler Mine. The bulkhead for the <br />Koehler Mine is part of the amended Consent Decree between WQCD and Sunnyside Gold Corporation, Case No. <br />94CV5459. The plans for the bulkhead were submitted by Gold King Mines Corporation and the work will be <br />conducted by Gold King Mines Corporation in accordance with the amended Consent Decree. <br />Location of the bulkhead was approximately 302 feet in from the portal. Preparations for concrete pour for the <br />bulkhead were near completion and appeared to meet or exceed the minimum requirements of the designs <br />prepared by John F. Abel, Jr., Mining Engineer, and approved by DMG on September 3, 2003. <br />The downstream face of the concrete form was constructed from 8 x 8 inch timbers, 2 x 12 and 2 x 6 inch lumbar <br />and 3/4 inch plywood, backed with heavy angle iron and well braced to rock bolts extending from the tunnel surface. <br />Upstream and downstream forms fitted intimately with irregularities in the tunnel surface. Tunnel ribs, floor and <br />back at the bulkhead location had been cleaned of loose rock, freshly exposed competent rock was observed at all <br />surfaces of the tunnel. <br />The final dimensions of the bulkhead, measured between the forms, was 12 feet Yz inch thick, 10 feet 6 inches <br />high, and 8 feet 5 inches wide. Rebar mat for the upstream face was constructed from #6 rebar on 12 inch centers. <br />Rebar mat for the downstream tace was constructed from #10 rebar on 14 inch centers. Spacing between the <br />rebar mats and concrete face exceeded the 3.5 inch minimum of the approved design. Metal washers were <br />mounted on the downstream face to mark locations for grout drill holes. A 4 inch diameter stainless steel bypass <br />pipe penetrated the bulkhead. Upstream and downstream rebar mats had 45 degree rebar reinforcement <br />surrounding the pipe penetration. The bypass pipe was fitted with one 3/8 inch thrust ring, located at the <br />approximate center between the bulkhead forms, and two seep rings, one on either side of the thrust ring. <br />Protrusions of rock in the tunnel surface were 1 foot or greater, a significant exceedence of the minimum 0.14 inch <br />tunnel irregularity specified in the design. The bypass pipe extended beyond the upstream bulkhead form to a <br />coffer dam.
