{Page 2) <br />MINE ID # OR PROSPECTING ID # M-2005-006 <br />INSPECTION DATE 07/13/2006 <br />INSPECTOR'S INITIALS DEG <br />This routine monitoring inspection of Permit Number M-2005-006 (Powderwash Pit #66) was conducted by Deb E. GBnima of <br />the DMG. Moffat County representative Marvin Moore was present during this inspection. The pit is located north on State <br />Highway 13 (36 miles), turn west on County Road 4 (33 miles), then turn south on County Road 66 for 5 miles to intersect with <br />County Road 668. This is near the northwest corner of the proposed permit area, which currently consists of approximately <br />30.85 acres, none of which are currently disturbed. <br />Mine entrance signs and permit boundary markers are all in place. There has been no active mining on this site yet, so work at <br />the time of inspection appeared to follow the guidelines and regulations set forth by the state and the mining and reclamation <br />plans for the pit, and no problems are noted far this site. The site is stable and no disturbance was observed outside of the <br />approved permit boundaries. For future reference, this site is correctly positioned in the DMG online GIS. <br />The only alteration to the site from its natural state is the grading of an interior permit boundary road that clearly defines the <br />perimeter of the site. The county representative mentioned a possible need for the installation of a culvert across an ephemeral <br />drainage (that runs south to north through the center of the site) as part of the construction of the haul road. However, no <br />formal plans have been set. This inspector stated that any changes to the mining plan would need to be submitted to the <br />Division prior to their commencement. <br />Responses to this inspection report should be directed to Deb E.
