s • <br />~~ (Page 2) <br />s <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE lO-S 99 INSPECTOR'S INITIALS BK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the Phase II and Phase III Leach pad <br />expansion area under construction on 10-5-99. Present during the inspection were, Mr. Scott <br />Lewis and Mr. Terry Mandziak of CC&V. Inspected the phase II LEACH PAD EXPANSION AREA. Most <br />of the geo membrane lined area was covered with drain cover. Clay liner was being placed on <br />the portion of the slope along the west, north west facing slopes of the rest of the phase <br />II expansion area. Golder QA/ QC personnel were present at the time of the inspection. Leak <br />Detection trench #8 was being extended along the west side of the expansion area as approved. <br />Inspected the temporary surface water diversion for the Phase III leach pad expansion area <br />is in place. The operator had placed appx. Every 29 feet a check dams and straw bales to <br />limit the potential for erosion. The secondary under drain south of the temporary surface <br />water diversion was in place. No ponding of water other than a wet spot was noted. Inspected <br />the phase II high volume and low volume solution collection area. <br />The following readings were taken from the Phase II HIGH VOLUME AND LOW VOLUME COLLECTION <br />PUMPS. LOW VOLUME PUMP #1 READ .43 FEET OF HEAD. <br />LOW VOLUME PUMP #2 READ .26 FEET OF HEAD <br />HIGH VOLUME PUMP #4 READ 29.8 FEET OF HEAD. <br />HIGH VOLUME PUMP #5 READ 31.2 FEET OF HEAD. <br />HIGH VOLUME PUMP $6 READ 35.9 FEET OF HEAD. <br />Inspected the Phase II under drain area. The temporary extension of the out let for the <br />discharges was being worked on at the time of the inspection. Both 24" pipes and the 4" pipe <br />were discharging at the time of the inspection. Inspected the ARD area, the structure that <br />will house the auxiliary tanks is taking shape, no major problem noted. AFTER THE INSPECTION, <br />A BRIEF DISCUSSION WAS HELD WITH Mr. Terry MANDZIAK regarding some changes the operator wants <br />to implement The operator wanted to change the drop structures for the phase III surface <br />water diversion structure from two to one. The Division notified them that as long as the <br />integrity and capacity of the diversion will not be compromised, the Division will not object <br />to the change and requested a written request with the appropriate map. Both Mr. Mandziak and <br />Mr. Lewis agreed to submit their request in writing.
