• (Page 3) • <br /> MINE ID # OR PROSPECTING ID 1 M-83-052 <br /> INSPECTION DATE 06/13/96 INSPECTOR'S INITIALS AJW CBM <br /> The following problems (PB) and/or possible violations (PV) (and suggested corrective <br /> actions) were identified during this inspection. The problems should be corrected by the <br /> dates given, or they will become possible violations. The possible violations should be <br /> corrected by the dates given to reduce their severity when considered by the Mined Land <br /> Reclamation Board. The inspector noted on the previous page should be notified of all <br /> corrective actions taken. <br /> PROBLEMS/POSSIBLE VIOLATIONS <br /> AND <br /> CORRECTIVE ACTIONS <br /> CORRECTION <br /> DATE <br /> 1. Failure to properly dispose of hazardous chemical 08/31/96 <br /> reagents or properly store and secure those reagents. PB or PV: PB <br /> TOPIC(S) : AT,_ <br /> Corrective actions are for the operator to remove the FN, SF <br /> chemical reagents listed on page two of this report or <br /> properly store them in an approved storage area. The <br /> financial warranty will be reviewed by the Division to <br /> verify that there is enough money available to complete <br /> the remaining reclamation inclusive of removal of these <br /> materials. If the operator chooses to remove the <br /> reagents, the bond calculation will be adjusted <br /> accordingly. In addition, if the materials are <br /> removed, proper tracking forms showing that these <br /> materials were transported and disposed of properly <br /> must be submitted to the Division. <br />
