• (Page 2) • <br /> MINE ID # OR PROSPECTING ID f M-83-052 <br /> INSPECTION DATE 06/13/96 INSPECTOR'S INITIALS AJW CBM <br /> OBSERVATIONS <br /> This was a routine inspection conducted by Tony Waldron and Carl Mount of the Division along <br /> with Orvie Zimmerman and Jim Harod of M & M, Ltd. The operation is currently inactive but <br /> there are plane to begin mining again. All areas where the placer deposit was mined have <br /> been backfilled with tailings and regraded. These areas have also been revegetated and <br /> appear to be doing very well. This area is south of the milling and office buildings. <br /> Identification signs and boundary markers were properly posted. The only problem noted is <br /> that there were 2 thirty gallon barrels of cyanide, 5 thirty gallon barrels of iron sulfate, <br /> and 100 pounds of Ferrous sulfate in the north part of the office/laboratory building. In <br /> addition, there was mixture of treatment chemicals (iron based) in a washdown sump in the <br /> same building. These materials should be removed and properly disposed of in accordance with <br /> current hazardous materials laws or locked up in a proper storage unit. The financial <br /> warranty for completing the remaining reclamation will be recalculated to include disposal <br /> costs for removing these materials along with the remaining tasks and the operator will be <br /> notified if there are any changes. <br /> I & E Contact Address cc: Carl Mount <br /> ❑ CE <br /> NAME Michael J.
