<br />~. <br />(Page 2) <br />HIVE ID # OR PROSPECTING ID # M-80-244 <br />INSPECTION DATE June 14. 1995 INSPECTOR'S INITIALS TAS <br />OBSERVATIONS <br />1. A partial inspection was conducted to observe progress of the Phase I leach pad <br />construction. Weather conditions were favorable and conducive to liner installation. <br />2. Installation of the clay liner was nearly complete. The entire pad area appeared to be <br />covered with the treated clay material. A large roller was being used for compacting the a- <br />foot lift. Golder Assoc. QaQc officer reported that all compaction teats run eo far have <br />either met or exceeded the required standard of 95fi Proctor density. <br />3. Ore was being loaded in the northwest quadrant of the pad area. Leaching of ore was also <br />in progress toward the southern extent ion of the loaded pad. <br />4. Construction of the 3-foot protective rock layer (above the 80-m1 liner) was observed in <br />the lower portion of the pad. Large haul trucks end dumped the finely crushed material, then <br />a bulldozer pushed the material eastward onto the bare liner. At the bottom of the working <br />face of the 3 foot layer there was a small fold, approximately 6 inches high, in the liner. <br />A worker was shoveling fine rock onto the back aide of the fold to ensure that the liner <br />would lie flat when loaded with rock. This technique seemed to be working well and was being <br />supervised by a QaQc officer from Golder Associates. <br />5. The Gundle crew was busy in the lower portion of the pad area cutting pieces of 80-m1 <br />liner to cover exposed areas and seaming sections of liner together with double fusion welds. <br />Pressure testing was being conducted by Gundle personnel on various seams. Several pressure <br />testa were observed. Results were favorable. <br />6. A seam of the 80-m1 liner connecting panels 50 and 26 was inspected. Pressure teats for <br />this seam were repeated on 6/12/95 and results were favorable. However, this seam was not <br />yet certified by Golder Associates QaQc officers. <br />7. Overall, the construction appeared to be well organized and running smoothly. Mr. <br />Wanstedt reported that completion of the pad construction was several weeks behind schedule <br />partly due to adverse weather during the previous months.
