(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-141 <br />INSPECTION DATE 8/05/02 INSPECTOR'S INITIALS ESC <br />OBSERVATIONS <br />This was an inspection of the Lyons Quarry conducted by Erica Crosby of the Division of <br />Minerals and Geology. Damian Murphy and Roy Hunt of Aggregate Industries were present <br />during the inspection. <br />The purpose of the inspection was to visit the area that will be amended into the permit <br />area. The operator proposes to add 15 acres to the Quarry, for a total area of 69 acres. <br />The addition of this land is to correct the disturbance conducted by the previous <br />operator, Golden's Andesite Mining. The permit was transferred from Golden's to Aggregate <br />Industries on June 4, 2002. The 15 acre addition will correct the problem associated with <br />mining outside the permit boundary, and take into account all mining related disturbance <br />associated with the site. The operator stated that a secondary amendment will be filed to <br />revise the mining and reclamation plan and add additional acreage to the permit area. <br />In particular, the operator is adding acreage to Area 4 located on the south end of the <br />property. The area includes the south end of the highwall and a number of rock piles and <br />overburden material created by Golden's prior to the transfer. The reclamation plan <br />proposed by the operator is similar to the approved plan. Changes to the mining and <br />reclamation plan will be filed under a separate amendment application. <br />The Division's main concern regarding the contents in the permit application is the <br />storage and location of topsoil. The previous operator pushed all topsoil and overburden <br />material over the face of the Quarry with a dozer where mining was not anticipated. Large <br />volumes of material exist on site, but the volume and location of such material is not <br />adequately documented in the permit. It appears from the site inspection that enough <br />material exists on site to achieve the required depth of 12" (on the original 27 acres) <br />and 6"-12" (on the 1983 amended 27 acres). The practice of pushing topsoil over the edge <br />of the quarry is not acceptable, and must be avoided by Aggregate Industries. <br />The Division's adequacy questions were sent to the operator under separate correspondence. <br />The Division's decision due date is September 9, 2002.
