-/ <br />•~ <br />(Page 2) <br />MINE ID ~ OR PA <br />INSPECTION DATfi <br />PELTING ID ~ N-80-244 <br />10-19-94 <br />INSPECTOR'S INITIALS ACS <br />This was a monitoring inspection of the phase 1 leach pad construction. I arrived at the <br />site at 7:00 p.m. in order to observed second shift construction activities, particularly <br />drain cover fill placement on the south aide of the pad. <br />There were 6 men working at the lower end of the drain cover placement dozer push. Two <br />dozers were operating on the drain cover this night. The six men were removing oversize and <br />working out wrinkles in the geomembrane in advance of the fill. Wrinkles are handled by <br />filling around them with a spade in advance of the fill such that they are covered by the <br />dozer push before they become large enough to fold over. <br />The operator had erected construction lights below the fill during the day shift, and the toe <br />of the fill was better illuminated than it had been the previous night. The laborers had <br />sufficient buckets with which to store oversize rocks, and had shovels which could be used <br />to break up agglomerated oversize. None of the painted PVC pipe grade (fill thickness) <br />markers were observed in the immediate vicinity of the fill being placed, however, it was <br />observed that the fill being pushed onto the liner was well over the required three foot <br />minimum. The operator ie reminded that the approved specifications require grade markers on <br />a 100' x 100• grid (more or less). <br />The pregnant solution storage area of the pad was inspected. Textured VLDPE has been <br />deployed east and south of the storm water sump above the toe berm. Approximately one third <br />of the textured VLDPE deployed has been covered by geotextile. <br />Drain cover fill placement was observed until approximately 9:45 p.m. I then returned to the <br />site at approximately midnight to observe operations from the Highway 67 overlook.
