(Page 2) <br />MINE ID # OR PROSPECTING ID # M87-143 <br />INSPECTION DATE 06/17/94 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division ae part of its monitoring program for 110(2) <br />permits and high-priority active metal mines, and to assess the adequacy of the bond amount. <br />Bond on this operation is currently $2800. The operator was contacted about this inspection <br />but was unable to be present for it. <br />This ie an underground operation on the site of a historic mine. There is one adit which is <br />gated and locked. There are still ore cart tracks in place on the main upper dump bench. <br />There are also some small structures located there, including office, several wooden sheds, <br />and equipment garage. Located on the main bench are about 10 ore carts, a pile of track, a <br />pile of pipe, fuel tanks and generator trailer. On the benches and dump slopes are <br />scatterings of various waste wood and timbers. There is also a large wooden mill structure, <br />and extensive old wooden cribbing at several locations in the permit area. <br />There was a large flow of water exiting the adit, heard though not seen. The toe of the <br />lower waste dump was observed to have a small flow of water, less than 5 gpm, which seeped <br />into the boggy area next to Gold Creek. A water sample was taken at this location. The <br />files contain a map which shows where the majority of the discharged water flows. The second <br />water sample was taken just below the top of the upper bench, south of the main cluster of <br />buildings. A flow of about 100-200 gpm was observed here. <br />Water samples were taken in triple-rinsed poly containers, plus pH and temperature was <br />measured. Samples were acid stabilized to pH of 2 or less. These were kept on ice until <br />delivered to EPA lab for analysis. Operator will be sent a copy of the result a. Being a <br />metal mine which is discharging water, this operation has the potential to be reclassified <br />as a "designated mining operation" or DMO. Operator will be sent a notification of this <br />determination soon. <br />There does not appear to be either a st ormwater management plan or any type of water <br />discharge permit for this operation. Operator will contact the Water Quality Control <br />Division, at the address shown and by the date listed on page 3, regarding the need for such <br />plan or permit. For all correspondence related to this, operator will supply a copy to this <br />Division.
