<br />MINE ID # OR PROSPECTING ID # M-79-195 <br />INSPECTION DATE 3/9/2001 <br />INSPECTOR'S INITIALS TAS <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. The operator has not posted the requested financial warranty <br />of $120,000 as requested in the Technical Revision No. 5 <br />approval letter dated December 23, 1999. <br />CORRECTIVE ACTIONS <br />The operator must clarify their intentions for Technical <br />Revision No. 5 on or before June 5, 2001 by completing one <br />of the following: A) submit a properly excecuted replacment <br />warranty of $120,000 or a supplemental warranty of $51,500; <br />B) submit a revised reclamation cost (in the form of a <br />Technical Revision) which includes a cost for demolition of <br />all plant site and office facilities currently present and <br />costs for bac kf filling & grading, topsoiling, and revegetation <br />based on the approved reclamation plan conditions (prior to <br />December 23, 1999). <br />2. The operator is conducting dewatering and dry mining <br />activities in the absence of an approved well permit and <br />an approved temporary substitute supply plan; both of <br />which are required by the Office of the State Engineer <br />prior to exposure of groundwater at the site. The operator <br />is also in violation of the mine plan which specifies that <br />no dewatering may occur until Suburban Sand & Gravel Co. <br />has provided evidence that they have satisfied all <br />appropriate requirements of the State Engineer. This is <br />particularly significant as the Division has received <br />four (4) citizen complaints from surface and groundwater <br />users in the Henderson Area since dewatering activities <br />have been implemented at Road Runners Rest II. <br />CORRECTIVE ACTIONS <br />Complete one of the following on or before June 5, 2001: <br />A) Provide written correspondence from the Office of the <br />State Engineer which verifies compliance with all of their <br />permitting requirements for existing dewatering, dry mining <br />and water management practices at the Road Runners Rest II; <br />(Page 4) • <br />June S, 2001 <br />PB or PV PB <br />TOPIC(S) FN <br />June 5, 2001 <br />PB or PV PV <br />TOPIC(S) HB,MP,ST <br />
