(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-Anna-nnn <br />INSPECTION DATE: n5/~na INSPECTOR=S INITIALS: _SAS- <br />1. Apre-operation inspection of the T-Bone Stone Quarry was conducted. The proposed quarry site is located <br />approximately 5 miles north of Lyons. Present during the inspection was Mr. Bill Tenore of T Bone Stone, Inc. <br />2. The applicant has proposed a 9.9 acre stone quarry to produce sandstone and Flagstone. Traditional quarry methods <br />will be employed to produce the stone products. The site will be reclaimed to rangeland. <br />3. There are three (3) areas that comprise the 9.9 acres: A) A 1.0 acre processing area, B) TBS 1 quarry area -includes <br />several historic quarry disturbances, and C) TBS 2 quarry area - no existing quarry disturbance. The processing area is a <br />grassy area with a heavy infestation of diffuse knapweed. Mr. Tenore affirmed that he is well aware of the need for weed <br />control for this noxious weed. The TBS 2 quarry area contained ponderosa pine, a sparse grass cover, and various shrubs <br />including chokecherry, vibes, and fringe sage. The TBS 1 quarry area contained a number of pre-law quarry disturbances <br />with highwalls averaging 10-12' in height and up to 100 linear feet. The TBS 1 quarry contained the following vegetation: <br />ponderosa pine, mountain mahogany, little bluestem, wild buckwheat, and mountain bluebells. <br />4. Photographs were taken of the proposed quarry site and are attached to this report. <br />5.
