<br />r <br />(PAGE 3) <br />MINE ID ~ OR PROSPECTING ID #: M-88-112 <br />INSPECTION DATE 4-2-92 <br />INSPECTOR'S INITIALS HHP, BK <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this inspection. The problems should <br />be corrected by the dates given, or they will become passible violations. The possible violations should be correc[ed by the dines given to reduce their uverity <br />when considered by the Mined land Reclamation Board. The inspector noted on the previous page should be no[ified o(all corrective actions taken. <br />1. Corrective Action Date: Mav 5. 1992 <br />Topic: AT(ACid/Toxic Materials) <br />PV(POSSible Violation) <br />As reported by the company, the levels of cyanide in the tailings Fonds are high. <br />The operator should take actions to return the cyanide levels in the ponds to some value <br />at or below the permitted levels of 3.8 Free Cyanide and 4.4 Total Cyanide. The <br />operator should take measures to mitigate the problem prior to re-starting the plant. <br />2. Corrective Action Date: Mav 5, 1992 <br />Topic: SF (Processing Facilities) <br />PB (Possible Violation) <br />The AVR system apparently failed to function properly, either as a result of cold <br />weather, high copper in the ore, or other unrelated function. <br />The operator should examine the AVR system and determine why t:he system failed. <br />Measures to mitigate the problem should be taken before returning the plant to <br />operation. <br />3. Corrective Action Date: Mav 5, 1992 <br />Topic: FW(Fish & Wildlife) <br />PB(Problem) <br />The dead gull, the report of dead ducks, and the deer caught in the fence may be <br />attributable to cyanide poisoning. <br />The operator should contact the Division of Wildlife regarding the deer to determine the <br />cause of death. With regard to the gull, the operator should report to the Division the <br />Results of the DOW tissue study. With regard to the reports of dead~9ucks, the Operator <br />should try to track down the source of the rumor, and determine whether ducks were <br />killed or not. Provided any ducks are found in the future, the operator should report <br />immediately such deaths to the Division of Wildlife and then to the 14LRD. Surveillance <br />of the site should continue until cyanide levels in the ponds are brought down to an <br />acceptable level. <br />4. Corrective Action Date: Mav 5, 1992 <br />Topic: SC(Erosion/Sedimentation) <br />PB(Problem) <br />Erosion along the flanks of the upper tailings impoundment exposed the underdrain and <br />the synthetic liner. The erosion control system around the upper tailings impoundment <br />is not substantial enough to contain future erosion. <br />Operator should re-dress the slopes around the tailings impoundment to cover the <br />underdrain and liner. Operator should install a sediment control term above the pond <br />to contain overland flow. <br />
