(Page 2)
MINE ID # OR PROSPECTING ID #: M-1984-014
INSPECTION DATE: 2-7-07
INSPECTOR'S INITIALS GRM
OBSERVATIONS
This inspection was conducted as part of the normal monitoring program established by the Colorado Division of
Reclamation, Mining and Safety for permitted sites. The primary reason for the inspection was a fact finding tour by
Division Geo-Chemist David Bird. Mr. Bird is now involved in the review of uranium mines and the Designated
Mining Operation appeals on these sites. Also, it provided staff an opportunity to verify the location and installation
of pizometers at the 4 current sites under DMO review. Glen Williams, representative for Cotter accompanied the
Division on site. Due to snow the site was observed from above along the county road.
The mine identification sign and affected area boundary mazkers are in place and in compliance with Rule 3.1.12.
The pizometer is located just north of the waste piles across from the main access road. The location of the well was
visually observed through binoculazs but does not appeaz on the accompanying photo well. Cotter will be submitting
data from this well with the other 3 sites prior to the Board Hearing on the DMO issues.
A[ this time there is no activity on the site. The inspector could not make meaningful observation of site conditions
due to snow cover and distance.
It should be noted that staff discovered that the last inspection of 10/23/06 contained an en•or for observations of this
site. For some unknown clerical mistake, observations for the JD-6 were inserted into the JD-8 Report. A
supplemental observations sheet will be done for that report.
Mr. Bird discussed and observed the general geology and hydrology of the mine azea with Mr. Williams.
No issues aze noted at this time.
Responses to this inspection report should be directed to Russ Means at the Division of Reclamation, Mining and
Safety, Grand Junction Field Office, 101 South 3`d Street, Room 301, Grand Junction, Colorado 81501, phone no.
970-241-1117.
I & E Contact Address cc:
NAME: Glen Williams
OPERATOR: Cotter Corporation
STREET: P.O.
