ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: ti+-IeRn-~aa <br />INSPECTION DATE: ~./R/nd INSPECTOROS INITIALS: ~~ <br />OBSERVATIONS <br />The Division conducted a construction progress and monitoring inspection of <br />the site on 6/8/04 Present during the inspection were Mark Tidquist and <br />Phil Barnes of CC&V.Inspection began at the sub station area, for an over <br />view of the Phase IVC leach pad expansion areas. Back fill and smooth <br />grading was taking place along the northern portion of the expansion area. <br />Inspected the Phase IVC western portion of the leach pad extension areas. <br />Clay, Geomembrane and drain cover were being deployed at the time of the <br />inspection. The deployed liners were walked and inspected; the deployed <br />liners appeared to meet the approved specifications. QA/QC personnel from <br />Golder Engineering, the design and QA/QC firm, were on site at the time of <br />the inspection, over seeing the deployments. <br />Inspected the Phase I, II, III and IV leach pad areas and recorded the <br />following readings from the Pregnant solution collection systems for the <br />high and low volumes. <br />Phase 2 high volume solution collection transducer # 1 was recorded at 20.3 <br /># 2 was at24.2 and #3 was at 22.9.Low volume Transducer # 1 was at .14 and <br /># 2 was at .47. Piezometer for the pond was at 34.1 feet <br />Phase II & III high volume solution collection transducer # 4 was recorded <br />at 28.4 #5 was at 23.5 and # 6 was at 29.2. Low volume transducer # 1 was <br />at .05 and # 2 was at .41. Piezometer for the pond was recorded at 29.5 <br />feet. <br />Phase IV high volume solution collection transducer # 7 was recorded at <br />21.8 # 8 was at 22.1 #9 was at 20.6 and # 10 was at 22.5. Low volume <br />transducer # 1 was recorded at 13.0 and # 2 was at 11.6 inches of <br />hydrostatic head.
