(Page 2)
MINE ID # OR PROSPECTING ID # M-1988-081
INSPECTION DATE 6/13/02
INSPECTOR'S INITIALS ESC
This was an inspection of the Phil Sheridan Lode conducted by Erica Crosby of the Division
of Minerals and Geology. The Division left a message with Rocky Conner regarding the site
inspection, however, the operator did not attend the inspection.
The site was identified with the mine name and permit number, and the fence line
delineated the permit boundary area. Explosive storage units were labeled and securely
locked. The portal and shaft areas were sealed. No water discharge was noted from the
portal area or the small draw adjacent to the portal location. A few structures remain on
site consisting of the hoist house, steelhead frame, and an office area.
The operator was reminded in past inspections (8-21-97 and 5-24-96) that if no mining or
reclamation was conducted on site within 180 consecutive days, the operator must file a
Notice of Temporary Cessation in accordance with Rule 1.13 of the Hard Rock/Metal Mining
Rules and Regulations of the Mined Land Reclamation Board. It is evident £rom the
inspection and past inspection reports that no mining or reclamation has been conducted in
some time. Therefore, the operator will need to file an application for Temporary
Cessation by August 16, 2002. A copy of the rules pertaining to filing this application
is attached. Please see page 3 for additional detail.
I & E Contact Address
^ CE
NAME Mr. Rocky Conner ^ BL
OPERATOR Comstock Mining & Refining ^ FS
STREET 999 Pinion Hills Dr. ^ HW
CITY/STATE/ZIP Carson City, NV 89701 ^ HMWMD (CH)
^ SE
^ WQCD (CH)
Cc: Carl Mount; DMG (via electronic transmission)
