(Page 2)
MINE ID # OR PROSPECTING ID #: ~-~nnn_~ SR
INSPECTION DATE: 05/~~ /n'~ INSPECTOR=S INITIALS: KAP
OBSERVATIONS
1. This inspection was conducted in response to two written complaints received by the Division of Minerals
and Geology conveying concern for preservation of the historical sites present within and outside of the permit
area and for land use related to the site. Present at the inspection was Kate Pickford, DMG. Two unsuccessful
attempts were made to contact Rocky Hoffschneider of Platte Sand and Gravel, LLC to notify him of the
inspection. Weather at the time of the inspection was clear, calm, and approximately 65 degrees.
2. The registered historical site to be protected which lies within the permit boundary is well marked and
surrounded by a split rail fence. The mining plan contained in the approved permit indicates that no mining will
occur within 200 feet of the historical site.
3. An anonymous complaint indicated that unmarked human graves may be present on the mining site outside
of the historical site. A copy of C.R.S. 24-80-1302 relating to the discovery of human remains was acquired
from the Colorado Historical Society and is enclosed with this report for reference.
4. A review of the status of Platte Sand and Gravel, LLC with the Colorado Secretary of State's office indicates
that currently the status is suspended (see the enclosure from the Secretary of State website). Platte Sand and
Gravel, LLC must provide documentation demonstrating the operator exists as an entity to do business in the
State of Colorado and has the financial ability to perform reclamation on site.
I & E Contact Address
NAME: Mr. Rocky Hoffschneider
OPERATOR: Platte Sand & Gravel LLC.
STREET: P.O.
