The operator has exceeded the approved limit of five 3/31/49 <br />acres of maximum disturbance (the limit set in TR- PB or PV: P~_ <br />O1), and has not submitted a recent map to addquately TOPIC(S): A_$_ <br />depict such present disturbance. ~, _, <br />CORRECTIVE ACTION: <br />The operator must prepare and submit a map showing <br />all present disturbance and activity, including the <br />pit, slope gradients, roads, stockpiles of gravel and <br />topsoil, topsoil stripped areas, areas being <br />reclaimed, or any other pertinent feature. The map <br />and/or its text must include sufficient information <br />to determine the amount of onsite (and offsite) <br />disturbance to be reclaimed. <br />This map, and its text if any, must be received by <br />the Division by the date shown at right.
