• (Page 2)

MINE ID # OR PROSPECTING ID #: M-19A~-nn7
INSPECTION DATE: nni21/nn
INSPECTOR'S INITIALS: C'I K
The following observations were made during an inspection in response to a complaint
received through the Larimer County Planning Department. It was believed that the
Operator had gone outside the boundaries of the permitted 2.5 acres.
1) The site is now adequately marked, with stakes and ribbons at the northwest and
northeast corners.
2) The possible excess site disturbance appears to be limited to the area along the
north side of the site, down the hillside from the permitted area. Shale rocks
have slid down the hillside toward the creek, but have been restrained from
choking or entering the creek before this point in time by a system of netting and
chain-link that extends across the bottom of the slide area.
3) When the site was inspected on May 9`°, a 15'long chicken-wire restraint area
between two trees had collapsed, leaving a hole in the restraint netting which
allowed rocks to tumble through and into the creek. This has now been repaired,
and the chicken-wire has been replaced with a chain-link fences.
4) Topsoil adequate to finish reclamation has been salvaged and stored at the site.
Further investigations of this site are in the works. We are attempting to determine
the level of pre-law disturbance at the site, and verify how much existed prior to
the permit being issued in 1986. We are also consulting with the County, and
possibly the Army Corps of Engineers, to determine if any further actions should be
taken with regard to the adjacent creek.
I & E Contact Address
NAME: Glenn Southwick
OPERATOR: Masonville Excavating
STREET: 220 W.
