(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-toRn-t ~t <br />INSPECTION DATE: ~4/~R/n'~ INSPECTORS INITIALS: ilAP <br />OBSERVATIONS <br />1. This inspection was conducted as part of the Division of Minerals and Geology's monitoring program. <br />Present at the inspection were Carl B. Mount and Kate Pickford, DMG, and Bruce Daniel, and Rod Scott, <br />Lafarge West, Inc. Weather at the time of the inspection was clear, calm, and approximately 40 degrees. <br />2. The appropriate sign was posted at the mine entrance and boundary markers indicated the permit <br />boundaries. Surface runoff appears to be confined to the site, and berms are in place adjacent to the river. <br />3. The western portion of the site is reclaimed with ponds and shores with appropriate slopes. The southern <br />portion of the site is mostly reclaimed, with some backfilling and seeding still to be completed. Along the south <br />edge, the access road is located on top of the berm adjacent to the river used for flood control. Mr. Daniels <br />indicated that the east end of the south pond will be backfilled. <br />4. Current mining activities are occurring in the eastern area of the site. Discharge from dewatering of the <br />active pond is into a series of adjacent ponds. Discharge into the river occurs rarely; only when the adjacent <br />ponds are full. <br />5. The area near the office, currently used to store stockpiled material has not yet been mined. The northwest <br />portion of the permit area remains undisturbed. <br />6. There is a diesel fuel tank located south of the mine office on the permit area. Currently the tank has a berm <br />around it, but no liner. <br />7.
