<br />MINE ID # OR PROSPECTING ID # M-98-013 <br />INSPECTION DATE 11/2/2000 <br />INSPECTOR'S INITIALS TAS <br />1. A site inspection was made of "The Shores" sand & gravel operation as part of an <br />investigation of various alluvial operations near the residence of John Roberts, located at <br />4789 WCR 24 3/4. The Division had received a written complaint from Mr. Roberts on <br />September 14, 2000 regarding declining water levels in his private lakes. (See attached <br />letter). Present during the site inspection was Mr. George Mizio, Firestone Plant <br />Superintendent. <br />2. The Shores operation is located approximately 1/3 mile southeast of Mr. Roberts property <br />and is bordered on the north by WCR 24 3/4 and on the west by WCR 9 3/4. The Shores <br />operation is also immediately south of the Rural ditch and Last Chance irrigation ditch. The <br />site is permitted with a financial warranty of $574,000 which is adequate for reclamation of <br />the existing disturbance. The site will be reclaimed to private recreation with a 112-acre <br />lake. <br />3. The Shores operation is active with the western portion of the permit area disturbed and <br />excavation currently in the Phase 1-3 area. A dewatering trench has been dug around the <br />perimeter of the pit disturbance and the operator has set up a water management system near <br />the processing area using a portion of the exposed groundwater, There is a series of fresh <br />water and process water ponds set up on a temporary basis near the plant facilities. Mr. <br />Mizio explained that all exposed groundwater and "process water" is contained on site and <br />there is currently no offsite discharge. Development and refinement of the water management <br />system at The Shores operation is still in process. Eventually, there will be an offsite <br />discharge as specified in the mine plan. <br />4. Current disturbance at The Shores operation appears to be in compliance with the mine <br />plan.
