The revoked permit carries the original idenrificarion number <br />of C-81-046. <br />2) On file at the Division of Minerals and GeOlogy.(Division), 1313 Sherman Street. Room 215. <br />Denver, CO 80203. <br />3) There Is no current document on file. <br />4) There is no coal extraction activity at the site. There are no curtent pormiis in place for this <br />activity. <br />5) There Is no record of such notification In curtent files. <br />6) Not applicable. No Subsidence monitoring was done at the site (see PAP, paragraph <br />2.05.6(6)). <br />7) There is no record of such monitoring or surveying in the current files. The Division is not <br />conducting such activities. <br />8) None known to have existed. <br />9) An original reclamation cost estimate starts on page 144 of the PAP.
