MINE ID # OR PROSPECTING ID #: M-faun-~aa <br />INSPECTION DATE: ~n-1a-m INSPECTOR'S INITIALS: aK <br />The Division conducted a monitoring and construction progress inspection of the Phase IV (B&C) areas on <br />10-18-02. Present during the inspection were, Tim Comer and Mark Tidquist of CCS:V. Prior to the site <br />inspection, brief discussions were held on the minor changes the Caissons and other backfilling and grading <br />issues were discussed. All the changes discussed were minor in nature and do not change the over all <br />performance of the pad. Under the Board Approved, Amendment # 8, the operator is given such latitudes <br />Division requested the changes and associated maps to be included once CC8cV has finalized the changes. <br />CC8:V, agreed to provide the requested information, once finalized. <br />Inspected the ARD building area. The shell for the add processing area is in place, no problem noted. <br />Insulation for the cyanide recovery tanks was being placed at the time of the inspection. Inspected the <br />certified and approved Phase IV (A) leach pad area. The second lift appears to be in place, no problem <br />noted. Continued the inspection to the Phase IV (B) AND (C) AREAS. The PHASE IV, PSSA AREA liner work is <br />continuing. A portion of the secondary liner along the South West portion measuring approximately 2o'x 20' <br />had to be cut open due to excess moisture below the liner. Golder QA/QC stated that prior to repair work <br />the clay liner would be repaired in accordance with the approved specification. No other problem noted. <br />Golder QA/ QC also explained that during the excavation of the low volume solution collection area, a wet <br />spot was noted and rather than connecting it to the under drain system, which would be practically <br />impossible, given the location, the water was routed below the elevation of the low volume solution <br />collection system area using under drain material. The area was inspected; no wet area was noted, AT THE <br />TIME OF THE INSPECTION. Golder QA/QC and resident engineer, Bob Nickels explained that due to the <br />minimal flow that was encountered, he did not believe that it would built any kind of measurable head in <br />the area. Division requested a written explanation and a small sketch showing the location and the <br />conducted remediation for the area. CC&V agreed to provide the requested information. <br />Back fill and grading work was continuing along the Phase N (B) AND (C) AREAS. No major problem noted. <br />Inspected the external pond and recorded the following readings from the low volume solution collection <br />transducers. # 1 was recorded at 15.9 and # 2 was at 13.7 inches of head, no problem noted. Pond was <br />holding some water. <br />Inspected the Phase I and II pregnant solution collection system and recorded the following readings from <br />the High and Low volume for Phase II transducers. <br />High Volume Transducer for Phase I had the following readings: # 1 was at 35.1, #2 was at 37.9 and # 3 <br />was at 37.4, (all readings are in feet of hydrostatic headj. Piezometer for the area was recorded at 44.1 <br />feet. <br />High Volume transducer for Phase II bad the following readings: #4 was at23,2; # 5 16.2 and # 6 was at <br />21,9. <br />Low volume transducer # 1 was recorded at .10 and # 2 was at .13 (ali readings are in feet of Hydrostatic <br />head.
