~ `' • • <br />(Page 21 <br />MINE ID # OR PROSPECTING ID # M-87.088 <br />INSPECTION DATE 3-19-96 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was apre-operational inspection of the expansion area proposed for the Hubbs Pit 1 12 conversion application currently under <br />review by the Division. The application proposes to expand the current 9.9 acre site to a 38.83 acre site which is to be mined in <br />four separate phases. The expansion areas lie south and east of the current site with the final phase located north of [he site, on <br />the opposite side of County Road 133. <br />Permanent manmade structures, within 200 feet of the proposed permit area, that were not identified in the conversion application <br />include overhead powerlines along the south side of County Road 133, a buried phone cable along the north side of County Road <br />133, and County Road 133 itself. Two irrigation laterals are located on site in addition to the irrigation ditch identified in the <br />application. One of the laterals is located on the west side of the phase 2 area. The operator's representative indicated that this <br />ditch is not functional and has not been used for many years. The ditch will be mined through and not reconstructed. The second <br />lateral is located along the east side of the phase 1 area and is functional. The lateral is to be moved to the east side of the phase <br />3 area. <br />Permanent manmade structures, on or within 200 feet of the permit area, that are to be affected must be addressed in exhibit S <br />of the application and include a written agreement for there relocation, repair or replacement. Permanent manmade structures, on <br />or within 200 feet of the permit area, that may be affected by the proposed mining plan will need to be addressed in Exhibit S of <br />the application and include either a damage agreement with the structure's owner, or a demonstration of measures to be taken to <br />ensure the siructuns will not be damaged. Structures identified include: fences (various types and conditions), overhead powerlines, <br />buried phone cable, county road, gas pipelines, gas wells, irrigation ditches and a stock pond. <br />It is unclear whether the fence on the north end of phase 4 belongs to the adjacent landowner, Ronald Gaddie, or to the Hubbs <br />Family Trust. The 3 or 4 strand barbed wire fence is currently non-functional due to its dilapidated condition. The gas line to be <br />relocated is approximately 3 - 4 inches in diameter and lies on top of the ground. The irrigation ditch that traverses phase 4 is <br />approximately 2 - 2.5 feet deep and wide. This ditch will be relocated east of the proposed permit area. <br />Sediment control fix stormwater runoff will need to be included in the application. These controls will need to be adequate to <br />minimize sediment transport to areas outside the permit boundary. A recommendation was made for installation of a sediment basin <br />located at the southwest side of the Phase 1 area while phases 1 - 3 are mined. Any sediment control measures should be described <br />in the application aitd delineated on the application mining plan map. The stormwater permit may need to be revised accordingly. <br />The Division would appreciate receiving a copy of the stormwater management plan for this operation. <br />Very few pinon/juniper trees will be removed due to the mining process. Approximately half of the phase 2 area is covered with <br />a sagebrush community.
