OPERATION STATUS: According to owner on phone call of June 1~, <br />1984 they have operated on this site since April 12, 1984 and were <br />actively operating on June 13, 1984. <br />2. PERSONS INVOLVED (NAMES, ADDRESSES, PHONE NO'S): See front page <br />3. INTENT TO COMPLY/DEGREE OF WILLFULNESS: Operator knew he nee ed a <br />permit and filed for a 112 on June 5, 1984 he then said in ph ne <br />conversation on June 12, 1984 he wanted a 111 but did not sup ly <br />information needed for a 111. <br />4. PROBABLE DURATION OF OPERATION - SUPPORTING EVIDE <br />See No. 1 above. <br />5. OTHER INFORMATION: Water table is exposed in 4 separate area . <br />There are 3 large crushed stockpiles on site.
