(Page 3) • <br />MINE ID # OR PROSPECTING ID # K-77-342 <br />INSPECTION DATE 2/23/96 INSPECTOR'S INITIALS ACS <br />The mill process water ie approximately neutral pH, and contains non-detectable to very low <br />concentrations of cyanide. <br />Actions taken by the Operator in response to this incident are detailed is the following <br />list: <br />1. The Operator inspected the flume at outfall 006 hourly from the time that <br />the loss of containment was discovered up until 5:00 a.m. on 2/22/96. Outfall 006 was then <br />inspected periodically during the day on 2/22/96. No flow in the flume was obeerved~during <br />these inspections. Apparently, the water discharged from the Ute Park Pump House soaked into <br />the ground and was absorbed by the snow which ie present in considerable abundance below the <br />pump house. Outfall 006 ie at the Henderson Mill property line, and is .approximately one <br />mile below the Ute Park Pump House. <br />2. The Operator has modified the procedures to be taken in the event of a <br />power outage to include manual closing of the Teledyne Valve in the Ute Park Pump House. <br />3. The Operator will connect the pneumatic valve to a much larger compressed <br />air reservoir that is available in the Ute Park Pump House. Once this connection le <br />completed, there will be a much longer lag-time between power outage and a critical drop in <br />back-pressure to the pneumatic valve that would cause the valve to open. <br />Conclusions drawn by DMO based on obsarvatioa^ during this inspection and on the Operator's <br />reports <br />1. Lose of containment of process solutions at the Ute Park Pump House did not <br />result in a discharge through outfall 006. There was no discharge of pro•:ees water to the <br />Williams Fork River. <br />2. The process solutions that were discharged from the Ute Pz~rk Pump House are <br />of relatively good quality, and were not of large enough quantity to have had a significant <br />impact on ground water or vegetation below the pump house. <br />3. Once the pneumatic valve in the Ute Park Pump House is connected to the <br />large compressed air tank that ie already available in the pump house, the Operator will have <br />implemented prudent and sufficient measures to prevent a recurrence of the incident described <br />in this report. <br />4. The Operator has met the notification and reporting requirements of Rules <br />8.1 and 8.2 of the Hard Rock/Metal Mining Rules of the Mined Land Reclame:tion Board. The <br />Operator's initial notification to DMG occurred approximately 24-hours after discharge from <br />the Ute Park Pump House was detected.
