N-80-244 <br />INSPECTION DATE 11-29-94 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />A monitoring inspection of the Phase 1 leach pad construction was conducted. <br />Floating tank heaters had been installed to melt ice that has formed in the pregnant solution <br />storage sump. The sump needs to be cleaned out and dried up to facilitate installation of <br />the pregnant solution recovery risers. <br />The earthworks contractor (Ames) was cutting the spillway into the "winterization" etormwater <br />and enowmelt detention berm. The geoeynthetics contractor (Candle) was installing <br />"sacrificial" VLDPE below and onto the "winterization" berm. Sacrificial liner ie being <br />installed for the purpose of protecting certifiable or specification composite liner from <br />stormwater or snowmelt impacts. Sacrificial VLDPE ie being installed on non-specification <br />soil liner, and ie not being pressure or vacuum tested. it was determined, through <br />discussions with Golder, that the sacrificial VLDPE panels installed are being recorded in <br />the installation documentation with an asterisk (e.g. SM-303* or *SH-303), and the extent of <br />certifiable or specification composite liner ie being surveyed. Also, the extent of <br />certifiable composite liner will be marked in the field, ostensibly with a yellow line, and <br />the sacrificial liner will not be covered by drain cover fill. It was observed that CQA <br />personnel were writing "To Be Removed 1995" on sacrificial liner pane le. <br />Drain cover placement was proceeding on the face of the Phase 1 tce berm. No problems were <br />noted. The location where leak collection pipes are booted through the upper VLDPE liner was <br />inspected, and found to be well completed. The upper VLDPE liner was carried up to the upper <br />anchor trench at the leak collection trench alignment, eo even in the unlikely event that the <br />pipe boots did leak, little problem would result because the boots are above the elevation <br />of cyanide application. <br />The earthworks contractor (Ames) was finish grading drain cover fill on the south aide of the <br />leach pad, and was removing the PVC pipe grade markers in the area being graded. <br />A location south of the pregnant solution sump was found to have a preponderance of oversize <br />rocks in the drain cover fill.
