The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />DATE <br />1. The Lyons Quarry is currently underbonded. The financial warranty _ <br />will need to be revised from its current amount of $142,800.00 to PB or PV:PB <br />$191,600.00. TOPIC(S):FN <br />CORRECTIVE ACTION: - <br />The operator will need to submit an additional $57,100.00 to be in <br />compliance with the Act and Rules and Regulations. The operator <br />will need to submit the $48,800.00 additional bond by June 24, 1999 <br />in order to avoid a possible violation of insufficient bond and <br />appearance before the Mined Land Reclamation Board at the July 27- <br />28, 1999 Board Hearing. <br />2. As required by Rule 3,1.12(2), permit boundary markers need to PB or PV:PB <br />be installed around the affected area of mining. TOPIC(S):BM <br />CORRECTIVE ACTION: <br />Submit to the Division proof of posting of the permit boundary <br />markers by July 30, 1999. <br />n <br />
