-,, ~ -. <br />MINE ID # OR PROSPECTING ID # M-77-141 <br />INSPECTION DATE 06/04/99 <br />(Page 2) <br />OBSERVATIONS <br />INSPECTOR'S INITIALS ESC <br />This was a follow-up inspection of the Lyons Pit conducted by Erica Crosby of the Colorado <br />Division of Minerals and Geology. Barbara Brunk of Rocky Mountain Consultants, Inc. was <br />present during the inspection. <br />The site was inspected on August 8, 1998 which resulted in reevaluation of the reclamation <br />performance bond held by the Division. On January 17, 1999, the operator informed the <br />Division that reclamation had been conducted on site including filling in the sedimentation <br />ponds, overburden has been replaced in Area 1, and topsoil has been replaced in Area 1. The <br />Division's inspection on June 4, 1999 confirmed that some reclamation has been completed, <br />therefore, the estimated volume of material was revised from 240,000 cy to 110,000 cy. <br />The revised bond totals $191,600.00. The Division currently hold $142,800.00, therefore, the <br />operator will need to submit an additional $48,800.00 to be in compliance with the Act and <br />Rules and Regulations. The operator will need to submit the $48,800.00 additional bond by <br />June 24, 1999 in order to avoid a possible violation of insufficient bond and appearance <br />before the Mined Land Reclamation Board at the July 27-28, 1999 Board Hearing. <br />The revised estimate is enclosed for your review. <br />In addition, the operator was required to post permit boundary markers and a weed management <br />plan. See page #3 for compliance due dates. It was also noted during the inspection that <br />groundwater was exposed in a small pond in the south end of the pit.
