R'~- <br />(Page 2) <br />MINE ID f OR PROSPECTING ID f M-80-244 <br />INSPECTION DATE 06/16/95 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This partial inspection was performed by the Division because the site is a high-priority <br />site, requiring frequent presence by Division staff during days of operation. The date and <br />time of this inspection were known to the operator. The operator's representative named on <br />page 1 of this report met this inspector at the security office, before proceeding to the <br />area of operations. <br />The main activity observed during the inspection was the preparation of part of the Arequa <br />Gulch surfaces prior to deployment of the VLDPE liner. Immediately above the edge of the <br />exposed liner, which was being tested, was the location of a berm built for runoff control. <br />This berm had been removed prior to the inspection that day. A quantity of oversize material <br />was exposed after the area had been regraded, necessitating hand-picking of rock before the <br />surface was ready to accept liner material. The surface and compaction of the area was also <br />irregular, which necessitated some amount of filling and rolling. <br />This inspector observed a crew picking such material for over an hour. The upper edge of the <br />liner was swept free of large angular material, both above and below the membrane. The crew <br />hand filled small voids in the clay layer, using screened material. This was subsequently <br />rolled several times by wheeled roller. After each pass, the voids observed were filled by <br />manual placement of screened fill. Rocks were again picked after each rolling. <br />After a satisfactory surface was prepared, crews spread the fine "flour" material as a light <br />covering. This was then rolled also. During this period, a water truck was wetting the <br />area. This not only settled the dust, but also made the clay material so wet ae to stick to <br />the roller drum. Once this was discovered, that equipment ceased operation temporarily (20 <br />to 30 minutes), until the surface was not sticky. The small voids created by rolling the <br />sticky material were filled by hand, and re-rolled. <br />The water truck, while descending the wetted hill slope on the south side of the gulch area <br />being prepared, braked to slow down, and slid on the slick clay. This had the effect of <br />creating shallow ruts, roughening the surface, and exposing rocky material. This elope was <br />beyond the area being intensively prepared that day, but this was pointed out to the operator <br />as an area to treat ae the operation proceeded uphill. <br />The bottom of the gulch, from 50 to 100 feet above the leading edge of the deployed VLDPE <br />liner, was (presumably) wetter and noticeably not well compacted. Initial passes with roller <br />and other equipment created ruts and ripples. This was also indicated to the operator during <br />the inspection. <br />The material being placed immediately on top of the VLDPE liner (to the north and south of <br />the area being prepared for liner deployment) was inspected for compliance with size <br />specification and depth. <br />If weather conditions indicated more rain was imminent, the operator assured this inspector <br />that the rain berm would be reconstructed to protect the liner and fill. <br />Other areas of the permit were not inspected.
