• fPage 21 • <br />MINE ID JI OR PROSPECTING ID 11 M-78-305 <br />INSPECTION DATE 06/05/97 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of 110 permits. The operator was notified about the <br />inspection but was unable to be present. At the time of the inspection, the entrance gate was closed and locked, and no personnel <br />was present. Late in the inspection, a single truck arrived and entered the site to load, then left again with a load of gravel, without <br />this inspector making contact with the driver. <br />The markers were generally observed to be present, though there were a few along the irregular boundary which could not be <br />located. The ID sign was posted at the entrance. Though there is no problem presently under the topic of "signs and markers" the <br />operator should ensure that all corner markers are maintained, especially those which consist of lath and are subject to falling down. <br />The permit area consists of several distinct areas of activity. The upper pit on the west is being mined down, and the highwall on <br />the west edge is kept graded to a slope of 2:1 along most of its length. The operator should ensure that enough material is allowed <br />to remain there so that no backfilling needs to occur, nor that grading is allowed to occur outside the boundary. Some of the <br />margins of the disturbance appears to be closely approaching the boundary. There are two topsoil stockpiles along the north edge <br />of the upper disturbance, which appear to be stable. <br />The lower level of the permit area is the processing and stockpiling area. It appears to be kept orderly. The scale and scalehouse <br />are still present, and there are a variety of different products stockpiled. There are two areas used for storing parts and materials, <br />one being just north of the scales, the other being a bit farther north, generally in the northeast corner. The Division understands <br />the utility of maintaining some of these materials onsite, but the operator must remember that the bond which is posted for this site <br />must be sufficient to cover the removal and proper disposal of all these items. There is currently a 513,300 bond, which will be <br />reviewed in the near future as to its adequacy. The operator will be sent the new figures at that time. If there is any significant <br />reclamation or housekeeping performed, the operator may influence the required bond if these activities are made known to this <br />off ice. <br />Concerning the more northerly storage area, this inspector observed numerous containers lying around which appeared to contain <br />various petroleum products. Specifically, what was noted included seven 55-gallon drums, one 16-gallon drum, three 5-gallon <br />buckets, mostly containing waste oil though some may have contained diesel too. There is evidence of spillage from many of these <br />containers, which seems to have been ongoing over time due to the appearance of the site and the depth of the contaminated soil. <br />The location of these drums, etc., is not bermed or lined to contain leaking or spilled fluids. The topic of "acid or toxic materials" <br />is noted as a problem on page one of this report, and the corrective action is on the last page. The barrels and contaminated soil <br />must be removed from the site. If future storage is desired onsite, adequate containment must be assured and bonded tor. <br />It should be brought to the operator's attention that two pieces of what appeared to be detonation cord (white colored, one with <br />a blasting cap?I were observed while performing this inspection. They were found on the bare fill slope east of the lower end of <br />the cat track used to access the upper pit. <br />There were no other items observed or further problems noted.
