(Page 21 <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 9124199 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS ~l ~ <br />This inspection occurred as part of DMG's routine monitoring plan for permitted operations and to <br />review the financial warranty adjustment requested under TR-02. Dave Diss accompanied the <br />DMG inspector throughout the inspection. The CM-25 Mine is approved for 7.34 acres affected <br />area and is located adjacent to the UMTRA tailings disposal project, approximately 2 miles SW of <br />Uravan. UMTRA reclamation activities have resulted in closure of the original mine access road. <br />UMTRA reclamation activities may have also disturbed lands originally included within the CM-25 <br />permit area. DMG holds 52,500 financial warranty. The permit status is intermittently active. <br />Public access to the portal is controlled by locked portal doors. <br />Signs & Markers Mine identification sign was posted at the previous mine entrance and needs <br />to be relocated to the new mine entrance. Some of the previously placed affected area perimeter <br />markers may have been removed by UMTRA reclamation activities and may require re-posting. <br />However, affected areas appeared sufficiently delineated, mining related disturbances appeared <br />to be confined to approved areas. <br />Facilities Affected areas totaled approximately 1.5 acres and included the following facilities: <br />Shop building - 40'L x 26'W x 12'H, Butler type building with concrete foundation and floor; <br />Compressor building - 20'L x 12'W x 10'H, with concrete floor; <br />Portal decline with shed - 60'L x 12'W x 12'H; <br />Vent hole -one 2' diameter vent with fan; and <br />Waste rock stockpile -apparently pre-dated the permit and possibly pre-law. DMG records do not <br />clarify whether the pre-existing waste rock stockpile has been disturbed by permitted mining <br />activities. Please clarify. UMTRA reclamation activities terminated at the toe of the waste rock <br />slope. If the waste rock stockpile has been disturbed by permitted activities, the reclamation plan <br />may need to be revised to ensure that final reclamation not adversely affect UMTRA reclamation. <br />Financial Warranty DMG will utilize the above information and permit documents to evaluate the <br />current cost of reclamation.
