.. - <br /> <br />INSPECTION REPORT - PAGE 2 <br />FILE N0. M-77-215 <br />DATE: October 17, 1985 <br />OBSERVATIONS OF IMPORTANCE: <br /> <br />1. The inspectors were accompanied by Mr. Jerry Danni and Mr. Richard <br />Bolling. No active mining on site at the time of inspection. <br />2. There appears to have been relatively recent barrel disposal at the toe <br />of the Windy Gulch waste rock area. <br />3. The upper culvert inlet on Windy Gulch is beginning to collect debris. <br />4. There are various barrels located throughout the mine site, and in <br />particular around the water treatment plant and between the mill and the <br />plant. <br />5. The upper diversion ditch located along the upper tailings retention <br />pond may not provide consistent down gradient flow. <br />6. The tailings flume to the upper retention area has numerous leaks. <br />RECOMMENDATIONS: <br />1. Barrels and trash must not be deposited in Windy Gulch. Please remove <br />those barrels which are of recent origin as soon as possible. <br />2. C]ean the upper culvert inlet of debris and vegetation and install a <br />trash rack at the culvert inlet. This should be done immediately to <br />prepare for spring runoff. <br />3. Please consolidate barrel storage on site, this applies to those barrels <br />as noted in No. 4 above (observation). <br />4. Please check the grade along the upper tailings pond diversion ditch and <br />correct as necessary to provide proper drainage. <br />5. The tailings flume must be checked and repaired to correct all leaks. <br />6. Please respond to each item in the recommendations section of this <br />report no later than November 20, 1985. <br />DB/mis <br />4629E <br />
