.. -

INSPECTION REPORT - PAGE 2
FILE N0. M-77-215
DATE: October 17, 1985
OBSERVATIONS OF IMPORTANCE:

1. The inspectors were accompanied by Mr. Jerry Danni and Mr. Richard
Bolling. No active mining on site at the time of inspection.
2. There appears to have been relatively recent barrel disposal at the toe
of the Windy Gulch waste rock area.
3. The upper culvert inlet on Windy Gulch is beginning to collect debris.
4. There are various barrels located throughout the mine site, and in
particular around the water treatment plant and between the mill and the
plant.
5. The upper diversion ditch located along the upper tailings retention
pond may not provide consistent down gradient flow.
6. The tailings flume to the upper retention area has numerous leaks.
RECOMMENDATIONS:
1. Barrels and trash must not be deposited in Windy Gulch. Please remove
those barrels which are of recent origin as soon as possible.
2. C]ean the upper culvert inlet of debris and vegetation and install a
trash rack at the culvert inlet. This should be done immediately to
prepare for spring runoff.
3. Please consolidate barrel storage on site, this applies to those barrels
as noted in No. 4 above (observation).
4. Please check the grade along the upper tailings pond diversion ditch and
correct as necessary to provide proper drainage.
5. The tailings flume must be checked and repaired to correct all leaks.
6. Please respond to each item in the recommendations section of this
report no later than November 20, 1985.
DB/mis
4629E
