To the best of my knowledge at this time, there were two construction <br />related incidents that contributed to the loss of the fresh water. <br />1. On August 24, 1998 there was a break in the PVC piping above the Hazel <br />5~1 A adit. This was quickly discovered because of a loss of water pressure at the <br />5 hose being used for compaction. The situation was corrected within 3¢ to 45. <br />minutes by shutting down the pumping system and running new pipe. It is <br />. o estimated that less ihan a 000 gallons of fresh water was released at a point <br />above the Hazel A and drained across the road and part way down Cash Gulch. <br />l~ ,y~e~° <br />2. The second incident began late on August 24, 1998 and was discovered <br />and corrected (by tightly closing the valve) early on August 25, 1998. It is <br />estimated that approximately 3000 gallons of fresh water was lost. As a <br />corrective action, we have now instituted a process of shutting down and visually <br />checking the water systems before leaving the site. Onca the new dam <br />construction has been completed, this part of the water system will be <br />abandoned and removed. <br />~ S'~ I~ <br />c~~~ <br />aaa~~gG . <br />
