• (Page 2) • <br />MINE I.D # OR PROSPECTING ID # M-94-117 <br />INSPECT ION DATE 07/29/99 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during a follow-up inspection of the site. Since the <br />Operator was not on the site at [he time, no inspection of the Mill interior could be made. <br />1) Gates and fencing for the site, preventing access from animals and unauthorized members <br />of th=_ public, as well as the required signs, are locked and in place. <br />2) The diversion trench above the pond is showing signs of erosion <br />3) A layer of fill has been placed and compacted on the downstream face of the dam, <br />minimizing erosion to the greatest possible extent at this time. However, the silt fence <br />below the downstream face has collapsed again, and the loose rocks on the face of the dam <br />do not appear to be stable. (PB1) <br />4) Piping still exists to transfer make-up water from the Hazel A to [he mill. The recent <br />rains have filled the pond with more stormwater runoff, but it appears to be at the same <br />height it was during the May inspection. Since DMG has not been notified of any change in <br />operations, it is assumed that the excess stormwater has been allowed to evaporate, rather <br />than being spray-applied to the landscape. <br />No new plans for final reclamation of the site have been submitted to DMG as yet. The <br />Operator intended to submit new proposals by late Suly, but no news has been forthcoming at <br />this time. Given concerns about the site, DMG would appreciate being notified as soon as <br />possible (within the month) about future plans. <br />No other problems were noted at this time. <br />I & E Contact Address <br />cc: Carl B.
