How A Party May Designate A Representative To APre-Hearing <br />Conference Or MLRB Hearing <br />INSTRUCTIONS <br />If, as a pazry, you will not be able to attend aPre-hearing Conference or Board hearing, you <br />may designate another person, such as a consultant, to represent your interests. However, in <br />order for the Pre-hearing Conference Officer or Board to recognize your representative, you <br />must complete the attached form. The form must include your notarized signature. For <br />items that do not apply, please write N/A in the blank space. <br />Your representative must present the original notarized form to the Pre-hearing <br />Conference Officer or Board at the time of the hearing. <br />
