The problems should be corrected by the <br />dates given, or they will becaoe possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Hined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />1. The mine identification sign did not contain the NLRB <br />permit number ae required by Rule 3.1.12 (1)(b) & (c) <br />Corrective actions are to: <br />1) Add "lII.RD Permit # H-77-479" to the <br />mine identification sign. <br />2) Submit a photograph to our office <br />which clearly shows that this was <br />completed. <br />2. The permit application does not contain a mining plan <br />map which outlines and labels previously disturbed <br />areas as required by Rule 6.3.5(2)(f). <br />Corrective actions are to: <br />1) Prepare a map of the affected area <br />that clearly delineates the <br />previously mined areas from the <br />affected areas that are included <br />under permit # M-77-479.
