(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2000-067 <br />INSPECTION DATE 5/6/03 <br />INSPECTOR'S INITIALS GRM <br />This inspection was preformed as part of the review process for a Technical Revision established by the Colorado Division of <br />Minerals and Geology. The primary purpose of this inspection was to verify that the map supplied with the requested Technical <br />Revision (TR-02) accurately depicts current conditions and proposed changes. The inspection was done as a matter of <br />convenience while the inspector was in the area. The Inspector attempted to contact the operator on the inspection date but <br />was unsuccessful. Since the inspector has been to the site recently, the operator s attendance was not felt to be necessary. <br />The TR will modify the Phasing of the operation to expand the Phase 1 from 10 to 18 acres. This expansion is needed to allow <br />a custom batch plant to be brought on site with appropriate working room. The site was noted being clean and free of debris. <br />Evidence of processed materials being moved was noted. No equipment of any kind was observed on site. <br />The TR-02 Map shows the new Phase 1 east boundary run <br />removed along this line and appears to delineate this edge. SI <br />in a straight line from north to south. Topsoil has been <br />same <br />At this time no erosion or storm water issues are noted. Control measures contain storm water on site and sediment barriers <br />such as hay bales in ditches were noted. <br />The operator is reminded that per CRS 3432.5-116(4)(8), topsoil must be protected from wind and water erosion if <br />stockpiled for more than 1 year. This issue is not listed as a oroblem at this time as it is not clear how long some of the <br />topsoil stockpiles and berms have been there.
